New Clients

     • Before your scheduled appointment, please fill out the client information sheet below in your preferred format. Email the completed form or bring it with you at your scheduled appointment time.

Massage Client Information (MS Word)

• Walk-ins are welcome and we will do what we can to accommodate you, but to ensure availability, reservations are advised.

• Due to the disturbance of others, we ask that all cell phones and pagers be turned off before entering the spa area.

• If you have limitations and require special needs, please inform us when scheduling your appointment. Your safety and comfort are our highest priority.

• If you schedule an appointment for anyone under the age of 18, a parent or guardian will be required to be in the treatment room with the therapist. This is to ensure not only the safety and comfort of the minor but also the therapist.

• All treatments at Harmonious Balance Spa are non-sexual. Improper behavior by either the client or staff are not tolerated. If this occurs, you will be asked to pay for your service and asked to leave the property.

• A 10% discount is given to the following employees: Educators, Nissan, State Farm, Military, and college students. Please let the therapist know if you are any of these so you can receive your discount.

Cancellation Policy

     • A 24 hour cancellation notice must be given to avoid a cancellation fee.

     • Late arrivals session will be shortened and may only receive the time remaining on their treatment slot. The therapist will determine if enough time remains to begin treatment, depending upon how late you arrived. Full session amount will be paid regardless of the time amount of the session actually done.

      • No shows are anyone who either forgets or consciously chooses to miss their appointment. They may be charged for the “missed” appointment.
